Few questions and was wondering if anyone had insight. Thanks in advance!

-- if a patient is unstable (ie: low BP in ED), do you place emergency orders like fluids BEFORE the physical exam?

-- in doing the uworld CCS, I haven't been asking for "follow-up" or repeat physical exams: is this necessary to do? i found that interval updates are automatically provided...?
 
Absolutely you place emergency orders before you see an unstable patient. You also only do the exam needed to address the problem. You get points off for doing a HEENT or neuro exam in someone you think may be having an MI for example.

To your second point, I think you are suppose to. I know I really messed up a couple of cases but my score on CCS was way lower than I expected it to be. USMLE will suggest a lot of follow up stuff for each patient. I bet a lot of it is overkill but maybe you need to do some of it? Not really sure. I can tell you from my N=1 if you do a good work up and get the right diagnosis on 10/12 cases you will pass without doing anything else.
 
I read it again. You asked about follow up physical exam. Never crossed my mind. Got automatic updates. Wouldn't waste your time on them.
