physio COPD question

patient with COPD, O2 sat 85%, which is increased?

2,3 BPG or Erythropoietin

porque?

aren't both increased? the guy's hypoxic-->signals renal cx to make epo. also, 2,3-BPG increases in hypoxic states.

what's the real answer?
 
My initial thought was that they would be both increased because they both respond to the same stimulus, tissue hypoxia. But since the hypoxia in this case is due to an effective respiratory acidosis, the blood levels of CO2, H+, etc., would all be increased already, causing a right shift just based on physiology. Therefore I would think that no 2,3-DPG needs to be synthesized for the right shift.

IMO Epo increased but that is a pretty tough question actually..
 
that was exactly my thinking...it appears that I glanced over the part of the question that says "which is increased in RBCs?" which effectively excludes erythropoietin since it is produced in the kidney.
