Wow, congrats on such great options! It sounds like you literally couldn't go wrong either way. Your cons aren't even really cons, tbh lol. Pitt students staying there is most likely largely influenced by preference, and $30k more over 4 years is not much at all for a future physician. I honestly think it'd be great to experience a different city. I've never been to Pittsburgh, but Philly is an awesome city. And being close to your partner will also be nice. Although you have your family support system at Pitt, so that's great too.
I think I vote for Penn because new experiences are nice and you already went to undergrad at Pitt, but Pitt and Penn are both amazing and extremely well-respected schools. Can't imagine you'd have trouble matching anywhere from either.
