The worst part of interview season has been not knowing how much to keep holding on hope for certain interviews. So:

Is it best to assume that 95% of invites go out during this first wave and then a few more invites might trickle out until November? Judging from the interview invites thread, a lot of top 30 programs send out their first wave of invites, and within the week most of the dates are filled up. Any advice on how it works based on last year or something?

Also, what should we expect of mid-tier places that are not in the top 30 but more in the 30-60 range, like georgetown. Are these the types of places that usually get a good number of cancellations so we can keep expecting interview invites until mid-december?

as you may expect the number of interviews offered later in the season depend on how popular the program is (and therefore how many cancellations they'll have)

speaking of gtown specifically, i got an invite the day after i sent in a LOI near the end of october. there are so many people applying to some of these programs that they have a tough time figuring out who's actually interested in matching there. if you don't hear from them in a week or so you should consider writing a LOI.

alternatively you can also be overestimating your competitiveness. feel free to PM if you want advice more specific to your situation.
