Pneumonia: Shunt vs V/Q mismatch

In UW, one place they describe the hypoxemia and increase A-a gradient due to pneumonia as a shunt :eek:

then in another question explanation, they say that "alveolar and interstitial inflammation lead to hypoxia and V/Q mismatch manifesting as A-a gradient"

which is it? what am i missing?

so what if question describes a patient who has pneumonia and it asks whether supplementation of O2 will help increase O2 sat?- what would one answer since does that not depend on whether we are talking about V/Q mismatch or shunt?......or did i completely miss the point
