Hopefully this thread doesn't devolve into something about how fellowships are stupid (they are), how much is a good starting salary (if it's as an associate it's not) or something along those lines....


Anyways today I did an I & D on one of my chicken's feet for bumblefoot. Not unlike a person. If it doesn't bleed it's no good. Well afterwords my wife asked if this makes me a clawdiatrist...

(a) That's not within your scope of practice. Kill that chicken before anyone finds out.
(b) awesome. You saved its foot. Now you have to debride its claws for the rest of it's life.
(c) apparently someone in the wound healing center had been trimming and applying potions to this chicken's foot for like the last 3 years!

Here's my go to:

Late one night a giant moth walks into a podiatrist's office. Goes up to the counter, presents his insurance card and says "I'd like an appointment with the doctor." The assistant leads him back to a room and the doctor walks in shortly.

Immediately the moth launches into a rant. "Doc, my life is in shambles. My job let me go, my kids don't respect me and I suspect my wife is having an affair. I applied for work at the local plant but I suspect my father in law blocked my application..."

The doctor cuts him off. "Whoa, what you need is a psychiatrist! I'm a podiatrist, why did you come in here??"

Moth stops and says: "I dunno, the light was on"
 
Ha. I like it. The other day I had an out of state person on "vacation" come to my office. Literally only wrote "foot pain, toes move weird" on intake.

The second I walk in she jumps up and starts screaming - I have nerve pain, I can't walk, my foot is broken, my ankle is fractured, I have foot drop.
Disentangling myself I order x-rays. Nothing.
Back in for more screaming.
No medication list - what medications are you on.
My Chinese doctor won't listen to me or prescribe strong enough medications.

So in residency I always viewed patients as - not my patients in the sense that - they are not mine to confront/have negative interactions with. They are my attendings' - I need to be polite etc. Anyway, I confronted her and said I didn't appreciate that she had written down none of her history, pointed out that she appeared to have numerous chronic problems which with more discussion had already been worked up by subspecialists if not to her satisfaction. I made some recommendations which she won't be implementing.

She ultimately departs, actually paying my full 99204 cost. But why. Why did you come to my office. Because the light was on.
