I would like to continue a basic science project that I've been invested in for a couple years now. I am interested in continuing the project, but would it be possible to go into lab like 5 hrs/week during clinical rotations?

Yes, for most rotations, as long as there is not a set time you need to be in the lab (can go at night, etc)

How can you do anything useful by working only 5 hours a week?
