i do believe it is possible... however, the chances of matching from an IM residency to pain is not that great... I am reluctant to say impossible, as there are pain programs for admission which will consider IM residents, meaning that it is a realistic possibility... but slim at the same time.....

For example you listed Spaulding Rehab. Another is UW Pain Fellowship Program. As the website states, "Three of these positions will be occupied by anesthesiology residency graduates; two positions are available for graduates of clinical specialties such as physical medicine and rehabilitation, neurology, psychiatry, internal medicine and family medicine. Training is provided for non-anesthesiologists in critical emergency airway and resuscitation management skills required of interventional pain practitioners." (http://depts.washington.edu/anesth/training/fellows/pain.shtml). It is a realistic possibility an IM resident is able to match into a pain fellowship program.

If you are interested in pain fellowship, I really do believe an IM background is very helpful for the field. But you also need to display an interest in the field by possibly doing research and/or becoming friendly with the anesthesia department at your institution.
