Is it possible to land a breast fellowship from a GS community program? I'm a 4th year, working on the residency application process and looking at places to apply to. I'm in Texas, would love to stay here, but will ultimately go anywhere. Any input?

One of my former Chief Residents from last year matched to a Breast fellowship. I'm at a community program in New York.

Sure; with the exception of the big name places like MD Anderson and Sloan, training at a big name university with research interests isn't really a factor in obtaining most breast fellowships. The reason its competitive at this point simply has to do with the fact that there are more people applying than there are positions...who knows what the situation will be in 5 years when you are ready to apply, but I wouldn't consider training at a community program a problem - there are different types of program for people with different interests (ie, there are breast fellowships at community programs).

Where would people with a breast fellowship go for a job? I mean do you have to work in an academic center or can you work in the community? I would assume working in an academic center would be competitive plus you have to be a good researcher.

I did a rotation where one of my surgeons was just breast and I loved it. It was surgery and you also had long term follow up (some people were following up after 10 years!). It made for great patient relationships and you could be an operator!

You have the same options as other surgeons:

private or group practice
academic practice
community hospital employee

and you also have dedicated breast centers, usually as part of a hospital center, with or without academic or teaching duties.

The majority of breast care takes place in the community and the majority of recent fellowship grads are working in the community, with a few in academic, research oriented tracks.
