hi there

i was wondering if anyone remembers what medications were prescribed after an impacted third molar is extracted (in india) to reduce swelling and trismus. i remember that there were some sort of enzyme tablets (2) but cannot recall what they were.

i mean apart from the usual NSAID/painkillers, anitbiotics and saline rinses and warm packs.

thanks.

Hi toothmail,
we used to prescribe serratio peptidase preparation to reduce swelling/edema after 3rd molar extractions.....that's what came immediately into my mind.if am wrong it's time I activated my hippocampus 😀 LOL

give the pt some dexamethasone too

y?is that wrong?

hi,
when i was doping my internship at govt dental hospital,i used to prescribe a combination of

1.amoxycillin (in case of penicillin allergic patients substitute with erythromicin)
2.biozobid(combination of muscle relaxant and serratio peptidase)
3.rofecoxib
4.saline gargling-the next day and an ice pack on the day of extraction

thanks guys.

dint some people also prescribe trypsin and chymotrypsin? or am i mistaken?

Hi, Toothmail:

Back in the old days, oral surgeons used to prescribe a papaya based enzyme(?) called papain to ease inflammation. It was ineffective, and they thought papain is better used to tenderize their steaks  Anyway, with the advent of NSAIDS the use of papain was totally disregarded.

Some really old surgeons used to premedicate patients with a laxative prior to the surgical procedure. Why? Because they thought that since the oral cavity is continuous with with the digestive tract, including the bowels, it would be a good idea to get rid of fecal material as a prophylactic measure. Those pesky fecal bacteria could climb up the digestive tract and find their way to the surgical site, eh? Does that make sense to you?

We've gone a long way since then!

hi toothmail, i believe wiztooth is right about the name of that enzyme. even i used to prescribe that enzyme tablets during my practice especially after impaction surgery. its really effective in reducing swelling and trismus, besides that it also helps in increasinig the efficacy of antibiotic in that inflammed area. i suppose wiztooth doesnt need to activate his hippocampus, haha.. peace, woeser..
