Hi,
Does anybody have any info about PRACTITIONER SELF-QUERY REPORT? California form for pharmacist shows that applicant has to mail that thing along with his/ her application for licensure exam NAPLEX.
Did any one send it before?

Yes, it is kind of extra background check. You have to submit itvwith your packet.
