Interviews won't get handed out 'til next October, so you'll just have to make the best judgment you can at that point on when she's going to pop. If you can, leave a week or so around her due date, but there's just no way you can plan when a baby is going to be born. Beyond saving some time around the due date, I wouldn't make your plans around your kid's birth. That sucks, but your only other choice is to turn down the interviews you get that you can't stick elsewhere - not exactly a great option, either.

Has anyone had their spouse pregnant and due to give birth during the residency interview season? How did you plan your interviews out?

If she is due sometime in November, schedule all your interviews in Dec/Jan. If she is due in December, all bets are off (she can deliver in Nov or December). You can leave 1-2 weeks in December without interviews, but you have to accept that you might

1) Have to cancel/resechedule an interview last minute because she goes into labor sooner than expected.

2) Have to miss the birth of your child because she goes into labor while you are away interviewing.

All you can do is make sure your top choice are scheduled at a relatively safe time relative to when she is likely to give birth. Keep in mind that you don't really know what your top choices are until you interview there (although you might have a pretty good idea if you do your homework).
 
Tough situation. I would just go into it knowing you may have to cancel at a moment's notice. One good strategy might be to schedule local interviews around her due date, so that you are in town at the most likely moment. Certainly you will have to call and cancel any interview held on D-day. It is absolutely unacceptable to miss your kid's birth for an interview. I think most places would be pretty understanding and try to fit you in on an alternate date.
