Does anyone know programs that will allow you too use a year of FP as your preliminary year?

A year of FP training, as well as peds, ob/gyn, EM, IM, surgery or transitional year can serve as your prelim year before starting in an advanced PM&R position.

FP programs usually don't have 1 year intern positions. They mostly got rid of those years ago

True. But if someone was unhappy in their FP residency, and wanted to transfer into an open PGY-2 rehab spot somewhere, then completing their intern year would satisfy the prelim year requirements.

Are there any programs that let you in without a prelim year? Are there programs that accept you with the prelim year already set for you or is this something you set up on your own then apply to PMR programs the next year?

When you register for the match and submit your rank order list: You could match into a categorical (PGY-1) 4 year program, where the prelim year is integrated into the PM&R residency. You could match into an advanced (PGY-2) PM&R slot, as well as a prelim year via your supplemental ROL. So your intern year would start July 2009, and your PM&R residency July 2010. Or – much more difficult and stressful – you could match into a prelim year only, and try to find an open rehab spot on your own during your internship.

Is there more competition for the categorical programs that begin with PGY-1 vs the advanced PGY-2 programs? I guess the obvious of getting to do all your residency in one area as opposed to possibly having to move after a year, but are there educational benefits as well? Would your categorical programs have more focused training beginning your PGY-1 year?

For the programs that offer both categorical and advanced postitions - categorical is probably more competitive. There are usually less slots available. Advantages would be you don’t have to move again, and you get acclimated to your institution’s system earlier. Some categorical programs do introduce you to rehab rotations in the PGY-1 year. Some categorical PGY-1 years are basically prelim medicine years in the same institution, and you don’t do any rehab until PGY-2.

In the grand scheme of things, doesn’t matter much.

I'm a 4th year applying to PMR and possibly doing a pediatric PMR fellowship. I would like to possibly do a pediatric prelim year, but was wondering if this would put me at too much of a disadvantage in managing adult inpt rehab, etc. during PGY2 year. I know transitional year is an option, but I hear those can be pretty competitive - so I'm trying to decide between prelim peds vs IM. Any thoughts?

I have no idea how competitive prelim peds is.

Most would argue that doing a prelim IM or a transitional year will serve you better during your PM&R residency, especially during your inpatient rotations. Some programs actually prefer an IM year over a transitional year. Plus there is always the possibility that during your rehab residency your interests may change. (although I hope it doesn’t – we need more good peds physiatrists). OTOH - no rehab program is going to rescind your spot if you do a prelim peds year.

Have you looked into the combined peds/PM&R programs?

Not sure which is more frightening - that PMR remembered that tidbit, or that Ludicolo actually found the appropriate thread!
