problem with dentpin:(

hay ..guys
I want to change my last name in my dentpin info. we can not change our last name online so I called ADA testing services .They ask me to email them at [email protected] with a proof of name and DOB.
I did the same no response yet ...I wanna take dates for exams .
anyone have any idea or ..anyone who change their last name or anything with the help of ADA testing services ..plz tell me how long will it take ?
