When placing a central line on an awake patient (like in pre-op) always use an US machine if possible (sterile technique of course), have the patient in trendelenburg, and HAVE THE PATIENT VALSALVA. This will cause the IJ to blow up like a baloon allowing you to quickly place your line and save you and the patient a lot of pain and frustration.
I suggest the following materials for board prep:
Baby Miller: This thing is solid for boards. Its deceptively dense & covers topics well.
Chattigan and Hall Anesthesia a comprehensive review: This thing is a must. know it cover to cover to earn big points on the exam. Its pretty straight forward and has great explinations.
MGH handbook: yup I said it. I read this thing all the time in the OR's.
Faust Anesthesiology Review: Another must. Know this bad boy for sure.
Michelle Star Anesthesiology Board Prep: I was suprised by how "on" this book was. Its set up sort of like your usmle first aid. simple explanations, pneumonics, etc. Great book. A few errors but overall I highly recommend it.
USMLE FIRST AID step 1: WTF?!?! don't worry. take 20 minutes and go over the stats crap we learned way back when. there were about 20 simple questions on the exam that caught me by suprise. I could have answered the majority of them with at least some clue if I had reviewed my FA stats section.
If you use the above your gonna kick some major tail. I suggest starting EARLY. What the hell, you gotta know this stuff anyways. You need only read an hour a day and your golden. after 3 years of pounding the above into yer noggin your going to be solid for the exam.
GENERAL KNOWLEDGE READING:
I prefer BARASH to MILLER.
Yao and Artusio (a favorite. read and grow strong young grasshoppah)
Reed's Clinical cases in anesthesia. (another favorite)
Morgan and Mikhail. (the bombsniggity)

Steward's Manual of Pediatric Anesthesia

Something I did after a year of frustration trying to remember what each attending likes and dislikes, and saving pearls (occasionally) given by attendings goes like this:
Buy a cheap spiral bound small notebook. Label the first section of the little book A-Z with several pages per letter. Then for every attending you work with you can put in how he/she wants the room set up for either just big cases (hearts, heads, spines, etc) or anything else that may be particular to them. This reduces frustration on your part trying to remember how that specific attending likes the drugs drawn up, syringes labeled, lines, etc etc AND makes you look like a superstar when they come in before the case and survey your setup. You can also note their common pimp topics, drugs they like to use and their dosages, etc etc.
The second section was saved for the random pearls I learned from various attendings. Credit was given to the attending who showed me but was simple, such as Dr.X's slick epidural placement, etc. Also other little tidbits like access codes to different places, key phone numbers, and whatever else you may like to save in a small, portable, central location.
I have had several lower level residents come to me and say " Hey Tim, I heard you had a little notebook for Dr Soandso, how does he like to do his hearts? Can I see your notebook?"
Some of my buddies told me I should get it printed off, and have the first 'useful' Senior Project. I appreciate the complement. It makes for an interesting read, saves me grief and time. It also stirs up old memories, some unpleasant, about my wonderful residency experience.
