Are there any programs that are 1. on probation or 2. on the verge of losing accredidation?

Go on the ACGME website and check under the 'accredited programs' link on all the programs you are interested in. They don't issue a list of the programs on probation, you will have to click your way through the site one program at a time.

If you applied to a program and they are either put on probation or are in the 'proposed probation' process, they actually have to tell you in writing about it.
