programs requiring supplemental application?

Does anyone has a list of program(s) that require supplemental application? i.e. an additional form (usually found on their GI fellowship page ) in addition to ERAS application?

I know Stanford, Duke, and Johns Hopkins did ask for it in the last cycle.
