"Literally" is used correctly so rarely. I recall back when Britney Spears cut off all her hair and the guy on the news goes "She is literally on a roller coaster to hell." And I'm thinking: Britney's riding a ride at Six Flags that goes through an interdimensional portal to the netherworld? Now that *is* news. But yes, there is literally an inverse relationship with resident number increase and fractions delivered.

COVID didnt help any Im sure. I saw an opening for one place but I heard that 2 fairly young ROs were fired due to low volumes

Major cities like Philly train wayyy too many doctors in general but RO its particularly bad. they arent going to into the BFE for jobs because they are all from metro areas themselves. LOL

Phily is a total dump. OVERRATED.
It’s not good for downtown living but if you can afford to live on the mainline it can be a nice life with a reasonable commute. Major metro areas are not really my thing but I’d rather live in a Philly suburb than LA or Houston. I wouldn’t take a pay cut or crappy job for any of them. To each their own.

Program hasn't successfully matched last two years I believe. Has a terrible reputation amongst applicants on the spreadsheet.

"The rumors about this program having a problem culture are absolutely true... Attendings + residents seemed like they overcompensated in trying to hide the malignancy in the program. Interviews esp. with PD, Chair were quite awkward, they seemed to barely have read my application. Must be "self-starter" in all aspects of residency, little support for education/didactics and research (residents go outside the program for research year projects it seems). I got the overwhelming feeling from my interview that residents were really just there for coverage and to do notes. Program has actively expanded recently despite problems in the field as a whole. Missing a lot of key disease site volume - GYN, GI, Peds, GU Brachy. Was told workload is heavy - busy call in all years (starting at 10 weeks in PGY2); Attending "strives" for 1:1, but can go to 2:1 if covering physician scientist. Recent residents go largely into PP, through stated as having academic "options." Board pass rate is 100%. Pediatrics must be at St. Jude in Memphis. //Avoid this program still stands// x4 to all of the above, good summary // not sure I agree with this stuff tbh, so hard to tell from zoom interviews // x basically came out and told me there were longstanding culture issues in the department when I asked... at least he was honest about it/ Any signs of improving? Or they don't even care? Really just need to decide whether DNR or not // DNR it."

It sounds like program expanded simply to increase coverage.

Place has always had malignant culture-
under Wally as well, and Adam dicker sounds worse. Somehow residents put in 12+hour days, but the volume of pts is not very high.

Currently there are an alltime high number of radiation oncology residents and total number of residents is up by 67% since 2001.

It’s crazy how controversial it is to even say it’s time to go back to 2010 numbers , or reduce by 25%, amongst those in “leadership.” You have the PD of MDACC getting pats on the back for just leaving 1 spot of 28 unfilled. Those in charge of this disaster are complete clowns.

I do think programs are starting to feel the pressure though. From what I've heard from a few PDs, the applicant pool is not a strong this year. Don't think that we are going to correct the oversupply all at once but I'm hopeful the resident class will shrink a bit this year and next year.

Nope. Hellpit places will take ANYBODY. Thats how these places operate. They need SOMEONE.

Can’t wait to see more of them on probation for boards failures in the near(ish) future. It’s rough going out there for mid tier places like us. We didn’t bother interviewing half of the applicants and then didn’t even rank 20% of the folks we did interview. Honestly, I wouldn’t be surprised to find out people get better qualified apps in the SOAP from people who didn’t get into a competitive specialty.

I honestly cant understand the “we just need somebody” mentality. PDs pride aside, bad residents suck. Unless you blindly sign their crappy notes with a weak @ss addendum at the bottom and straight up ignore their contours they make life worse, not better.

They will justify these ANYBODYs and lower quality applicants as "more diverse," "having different life experiences," and "extremely motivated and willing" - anything to justify what they know is a decrease in quality.

Attendings at these places are drunk on 100% coverage.

You are right but its a piss poor calculation in my mind. If they have a half way decent patient volume the GME funding should be negligible as part of the operating budget and not worth the trouble. I mean, I could pick up a couple shifts at the local Taco Bell after work a couple nights each week for some extra income if I really wanted to...

At these places residents are used for cheap non-unionized, no 401k match sweatshop cheap labour. the residents in some places even have a worst health plan than the attendings. Residents don’t leave exactly when their shifts end and demand no over time. When the “department” is so drunk and hooked on this sort of dynamic, there is widespread panic at the thought of not having just ANYONE. hellpit places will hellpit. This is the crux of why this issue will never be fixed. The ACGME needs to aggresively come after hellpit places on merits, board failures, etc and shut them down. Millions are in jail for a crack pipe. why do these people get so many chances?

This is why the only thing that will work is program closure and official contraction of spots, which of course will never be done because ever chair thinks their program is awesome (I'm thinking of Dan Spratt's post saying he will make Case Western a top 10 program through shear force of will, lol).

If by withdrew you mean they were forcibly shut down by the ACGME for being a malignant program, then yes they were withdrawn

Many more programs need to be Cornell'd in order for the specialty to return to good and robust health circa 2000-2015

Had heard multiple rumors that chair is crazy.
If she is she hides it well. And I highly doubt an otherwise respected medical school in a prime location would let that stop them from collecting GME funding and keeping the prestige of having a residency program. I honestly have no idea why they did it. It would be great to get first hand info.

Rumors came from when she was at NYU. Everyone agrees she is a great scientist.
 
Good friends with a former resident at Cornell.

IIRC, they were shut down after having back to back negative reviews by ACGME surveys.

Complete lack of support for the residents across the board with really no willingness to change.

Getting research time very difficult. Friend was giving a single day to go to ASTRO then expected to return (despite presenting poster).

Clinical experience pretty uneven with a ridiculous emphasis on breast. Seriously, after 18 months of residency 10 were breast.

During COVID, hospital was given Carte Blanche to do with residents as they pleased. A PGY2 was sent to ICU night float, others to manage the floor which were basically ICU.

It was a malignant hell pit. Enough said. Many more just as bad in list. “No support for residents” is a common theme in these hellpits. Refer to the hellpit thread for a refresher if you need it.

Could say the same thing about ny Methodist, downstate.

Also the same about wvu needing a program when Pittsburgh isn't that far away.... Why does cleveland/Detroit/Pittsburgh need more than one program at all honestly?

Note, it looks like the "new" NY Methodist spot that got a lot of comments was a spot that was already approved since at least 2019 but just never filled. I guess they are now actively trying to fill that.

One program that may have issues with the new RRC requirements is the University of Washington due to how they rotate their residents at different sites.

A total of 815 potential training spots of which 771 have residents enrolled as of 1/12/2022. So 771 residents training works out to 192.75 grads/year.

Looking at the above list clearly all the 4 and 5 resident programs could be closed down but we would still be about 180 grads/year in that scenario.

Really hard to see how we ever get down to a number that would represent just replacing retiring docs at lets say 125 grads/year.
