Pt. Monitoring Form for ICU/Critical Care pts?

Does anyone have a monitoring form that they would be willing to share that would help track trends geared towards patients in an ICU? I have a monitoring form, but it's a pdf [Read: I can't edit it] and it doesn't include some things that I would like to be tracking like O2 sats, I/Os, etc. etc.
