you'll be fine, especially if you're applying MD only and not mstp. That said, lots of people applying mstp don't have a publication, nor have they even submitted one. As long as you don't drone on to the schools that you got rejected (wouldn't use that word in an interview), then I see no issue. The positive req's will be much more informative to the schools.

How could a rejected publication hurt you? As far as adcoms know, that rejected work was nonexistent unless you bring it up which you obviously will not be doing
 
You're well ahead of the game, so don't worry about the rejection.

Can you find a different journal to submit it to?
 
You have absolutely nothing to worry about. I'm sure you know that publications get rejected all the time by even some of the brightest scientists out there.

PI or the first author would be crazy if they didn't. Really the only thing you lose with rejection is time and the possibility of getting scooped, aside from prestige points of going to a slightly lower journal (although, outside of going from science/cell/nature on down, even this is fairly negligible as long as it is reputable).

However, you do gain what you have learned from the reviewers.
