My long-term girlfriend (dating 7 years; long-distance in college) are both applying to medical school together. We are both 21 and have similar stats (4.0, 520+) and both have research-focused applications. We have been long-distance for the last 3 years and would love to go to medical school together. Would it be advisable to write in additional comments that we would like to be considered together?

I don't expect that it would help. If one of you gets an offer and the other doesn't have an offer in the immediate vicinity (you don't hold offers from different schools in the same city), you might ask at that point if your partner can be considered.... Given your stats, I'd expect that you may well have good luck in NY, Boston, Chicago, DC/Baltimore, Philly, St. Louis, New Haven or other metro areas with more than one medical school.

Is there a date?

I've seen people mention it if you have additional comments. No guarantee admissions will be able to do anything with it. I agree with Lizzy that you should focus on locations with multiple medical schools to maximize your options. You should also have contingency plans such as PhD or masters programs.

You should discuss the impact on financial aid and insurance should your relationship go legal.
