why the crystals accumulate in joint of big toe first as compared other joints of body?????please do answer
 
because the crystal be come soluble in hot but in the periferal body it can not soluble and collect there and you cand find an animation here
 
i swear someone asked this in my class and the given answer was.....gravity...

of course that would explain why the toe as opposed to the finger....

Yes, you really have to break the word down to understand what is meant here.

peri- means around or near

feral means wild or animal-like

So we come to the meaning of periferal: near the wild. We walk on our feet and touch with our hands so they are nearest to nature or the wild.

oh my english is not very good it is a mistake i mean *** peripheral****

I had 'ya covered. Anyways . . . I wouldn't know how to say almost anything in almost any other language so even knowing 2+ languages is impressive to me.

well do you think that the temperature difference is enough.......and also if temperature difference is the cause then there are oter joints too like other toes...?i think there is something about the type of joint of big toe.😕

They think it's because of temperature difference that it occurs in the big toe and other places away from the center of the body, such as the instep and ankle, but as far as I know they're not 100% sure why it occurs there so often. But they also think it occurs in the big toe because trauma and physical stress can precipitate a gouty attack, and our big toes are always under stress because we push off with them when we walk, so it gets a lot of wear and tear. This was told to me today by a rheumatologist who was my group facilitator for one of our "how to be a good doctor" classes, so I trust what she says. Plus, it was in our lecture notes.

Gout occurs most commonly in the big toe because uric acid is sensitive to temperature changes. At cooler temperatures, uric acid turns into crystals. Since the toe is the part of the body that is furthest from the heart, it's also the coolest part of the body—and, thus, the most likely target of gout. However, gout can affect any joint in the body.

It is largely the temperature difference, but I believe what the rheumatologist said regarding stresses to the foot/toe is also correct even though I haven't read this anywhere else. I believe it because I had a gout attack the day after a fairly rigorous run that put a good deal of stress on my foot. Which of course confused me since I thought I had fractured something, and my own PCP kinda ignored my insistence that the exercise had something to do with the attack and instead suggested it was the 1 glass of wine I'd had the previous week (btw, red wine isn't actually a big gout trigger, drinking lots of beer is though). In retrospect I think the fact that I had twice vastly overeaten huge amounts of red meat at Churrascaria Plataforma in the previous month probably had the most to do with it, lol.

But yeah, since the blood cools down there when you sleep the crystals tend to precipitate out in your extremities.

And trust me...it hurts a ridiculous amount. I've injured myself in many varied ways, but...gout is the kind of pain that won't just wake you up, but will wake you up screaming.

well i have considered a lot of things that i should be opting in future but never rheumatologist......i have not yet made up a mind because i think its immature to decide cause i have not yet visited any wards........still considering alot more....:laugh:
