Question about reconstituting Doribax 500mg Vials

I work at a small independent pharmacy, that also does some home IV infusion therapy. We recently just started a pt on Doribax and when reconstituting the vials it looks quite clowdy and there are a lot of particulates. Is this normal? I know it is a suspension when reconstituted, but frankly it looks like s**t. There is a considerable amount of particulates stuck to the inside of the vial. Any helpful comments will be appreciated. Thanks.

Yup, very normal. I usually reconstitute with 6cc of NS, shake the vial and shoot that into the bag. That'll leave you with a few drops at the bottom...shoot another 4cc into the vial, and that should mop up the remainder. The reason I use fixed amounts is because we normally only reconstitute if its going to be a 250mg dose and we need to split a 500mg bag.

Thanks, that is exactly what we were going to do. I told the tech to do 6ml with a 4ml wash. Thanks for the input. If anyone else has any ideas, let me know.

I have noticed that many of the carbapenems look like this. Ertapenem and meropenem also look cloudy.

The dose was 1g Q12, otherwise I would have mini-bag'd it up.
