I was wondering, does the traditional rotating internship rule in the 5 states apply to DOs doing ACGME or AOA residencies, or does it only apply to AOA residencies?

Four states now. Applies to all DOs who want to practice in those four states, AOA or ACGME residency notwithstanding.

If you do an MD residency in one of the four states, resolution 42 gets you out of that requirement.

In practice (at least in PA), just about everyone gets approved. So, don't worry about it

Although this may very well be true for some, even most specialties, it is not completely accurate. For instance, it can be quite difficult to get approved in surgical specialties. Most ACGME surgical interns do not do any rotations outside of surgery. You would have to beg your program to allow you to do an obgyn month, fam med month, internal med month, etc (6 outside months total) to get approved through resolution 42...

In 2001-2004, only 1% of applications (4 people) for resolution 42 were rejected (source). From 01-06 it actually got better (9 people rejected out of 937) (source).

This data is for all resolution 42 applications in all of the 'special' states.
