I live here in Huntington and work at Cabell Huntington Hospital, so I am pretty familiar with the school. I applied there this year as well. Are you applying, or have you been accepted? If you have been accepted, congratulations to you!
A good portion of the third and fourth year rotations are in Cabell Huntington Hospital, located about 10 miles from the medical school, the VA hospital, which is next door to the medical school, and St. Mary's Hospital, located about 2 miles from Cabell Huntington. From my understanding, you have the option to have some "away" rotations as well. I know a couple of my high school friends that went there have done some rural FP rotations in our hometown of Logan, which is about 70 miles south of Huntington. I believe that you can also do some rotations at CAMC in Charleston, which is about 50 miles due east of Huntington.
I work around a lot of the internal medicine and family practice docs at Cabell Huntington. I assume it is like every other teaching hospital in the country. The attending, or resident, has a group of 4 to 6 students that follow him/her around and provides teaching and other things. My friends have told me that the rotations are not too bad at Marshall, but they have made some comments about two rotations in particular. Anyway, I hope this helps you some. I went to Marshall for undergrad and graduate school. It is a nice area with new malls, theaters, golf courses, etc. The Marshall University football team is also a plus. See you round.
