I am a boarded pain/ anesthesia currently working at a diagnostic center.
I do pain injections at their facility.
This is in Texas.
I’ve also been doing joint arthrograms - just injecting the contrast.
The radiologists read the MRI arthrograms.

Not sure what has happened to their radiologists but at this time they’re not able to recruit anyone. So they’re asking me to prescribe sedation for claustrophobia. The medication will be administered by RN or RT.

They’re asking me to prescribe Xanax 1-2 tabs as a standing order for mri. I don’t have an issue with this per se, as I would write this for my own patients.

But they’re also wanting a standing order for other facilities and other locations where I’m not working out of.

I don’t know how appropriate this is. I told them I need to study and investigate this more. Because I’m concerned that I write an order but I’m not immediately available.

How do radiologists typically deal with this?

I’m not experienced in this?

Will this impact my malpractice insurance? Do I need forms/ arrangement and documentation?

If I am not seeing a patient myself, i am prescribing nothing. We do not have standing orders for scheduled meds for all MRI patients.

Not sure if you'll find an answer, since your exact situation isn't that common.

The most common situation is that either the referring physician, or a rad on site acts as the prescriber.

I've heard of a few set ups like yours where it's not the rad doing the procedures, but in those cases it's a single facility/site.

I haven't heard of physicians issuing standing orders for all sites.
I personally would not be comfortable having a standing order at facilities I have never worked at myself. You have no idea what their capabilities are, the people who work there, if there is an ER doc around or someone else who can help out if needed, etc.
