you need two lesions for the WREB. Use both! Unless, you already have the other lesion. If so I would choose the premolar.

Use Caries indicttor, if the ridge is sound, but has an occusal distal pit, you have to restor that too. On you premolar make you have have contact with opposing arch. if you do have countct, make sure the contacts are not on the cups, that would be consider cusp to cusp contact.
and If you have any contact with adjacent tooth, even if it is rotated, just mention in your paper that the contact point would be different since the tooth is rotated. let me know if I can be any help
