I honestly have no idea because it was 2 years ago and I've since transferred to a different campus under the same university.

If the letter didn't get sent to anyone, then you've been given a second chance and you don't have to report it (from my understanding) because it's not an IA - your prof just gave you a 0 and everyone moved on. If the letter made it to the dean's office, you would have to have a chat with your dean, because at that point, its probably something that needs to be reported or at least brought up with your premed advisor.
 
What if it did get sent to the Dean but they didn't do anything? I've never gotten an email or letter or any sort of communication outlining that I have anything on my file. Unless they just put it on there and never notified me...

That might be something worth talking to your premed advisor/the dean about. Double check what AMCAS says in their rules regarding reportable IAs.
 
I feel like you would have received some official notice from the school if anything formal had been filed against you/added to your transcript. This prof probably understands how lethal it would be to report you and has essentially given you a hardcore warning. I would not report this and I think you will be fine.

You should be able to review your academic file with an advisor.

In addition, you can play the "what if" game all day, but from what you have told us it sounds like something you aren't required to report.
