Questions about TOEFL reporting

Hi all,

During placing an order for TOEFL, there is an option of informing 4 institutes for free.

I tried to select CAAPID using their code which is b451, then under select school type, there are other selections as follows
1- undergraduate
2- graduate
3- graduate school of management
4- Law school

Just to let you know I am willing to apply for "internationally trained dentist" programs.
Which selections do I choose ?

What are these programs considered to be? , Are they graduate or undergraduate?

All dental schools are considered graduate schools (at least in the US.)
