I have been looking at the residency programs in gas recently and have a few questions I?m hoping some of you can answer.

First of all, I was looking at the program at the Cleveland Clinic (I happen to live in Cleveland) and it appears to have one rather general PGY-1 internship year followed by three years of anesthesia training (as CA-1, CA-2, CA-3).

Is this program typical of anesthesia programs (ie, 1 + 3 years) nationwide? That is, do most programs have the required non-gas internship year? Is this all the post graduate training an MDA needs to provide anesthetic services in most situations, or do MOST MDA?s go on to do a fellowship?

The fellowships offered are usually 1 or 2 years (two years in the case of certain cardio-thoracic fellowships). Am I right in understanding that these are in addition to the general four years of residency.

Many programs integrate a pgy1 year but not all.
I'm not even sure most do. It's usually a limited number so if they have 15 spots for anesthesia pgy2-4, they may only offer 5 full 4 year spots. We're starting it at our program because of interest by applicants and also to allow us to structure an internship more useful for an anethesiology resident.

4 years is all you need to pass gas. 15 years ago it was only 3.
Most do not go into fellowships at this time.

The fellowships you see are in addition to 4 years. There are many CT fellowships that are 1 year. Hopkins is 2 years unless you are one of their residents, then they integrate your CA3 year as part of your fellowship so it's only an extra year. Depends on the program. There are many MDAs out there doing hearts without a felloship.
 
If many (most) gas programs don't integrate a PGY-1 internship year into thier programs, and yet it takes four years to certify), where do you get this internship year for those programs that do not integrate it? Do you "match" for only a 1 year internship, or do you match for the gas residency and find the internship some other way?

Yes, if the program doesn't provide a pgy-1, you have
to match in one on your one. You need to interview at them
separately and rank them on a separate list. Most medicine/pediatrics/surgery programs allot preliminary pgy-1 spots or you can do a transitional year which is more of a traditional rotating internship.

What is a "transition" year? Is that different from an internship year?

And while we are at it, what does it mean to "scramble"?

Transitional year is a traditional rotating internship. Ususally 4 months IM, 1 month ER, 1 month FP, 1 month OB or Peds, and 5 elective months. Results may vary, but this is close to universal.
