I have found a dentist to shadow, but when I do, what info should I request from him? I know that I should ask questions about dentistry, but should I also ask for his contact info so that I could have proof that I shadowed him?

I did my first shadow today and it was a great experience. Dr. Phillips let me follow him around, told me things that I didn't even think to ask, and didn't grill me about prior knowledge. From my experience, I would just ask the things which you genuinely are interested in the answer. I pretty much stood in a corner while he explained procedures and told me about his patients. It was a great experience.
I did ask him, since he was younger, which 4 year school he went to for undergrad. (Our school in town is only starting its 4 year program.)
I asked him what influenced his decision for his particular specialty.
Things along those lines. I didn't want to overload him or me on the first day. 🙂
As far as proof about shadowing, I haven't found a way to bring that up in conversation.

Here's what I did. I had an excel sheet printout with date, hours, dentist, and a spot for his/her signature. That way it was pretty much on one-two sheet for schools that requried proof. I also had a personal record of notes from my experience as well as their various specialties.
