I initially thought married filing separately would make more sense with the PAYE loan repayment, but if my math is correct, it seems it would be beneficial to file jointly.

For PAYE this year, after the 150% poverty level deduction, it will be zero payment either way.

Even next year when we're both working the entire year the PAYE increase is $2500 for filing jointly, but I imagine the tax benefit for filing jointly will be >$2500. So it seems it may be best to file jointly for my situation throughout my entire residency

Yup married filing separately is pretty punitive on its own.

If you have a huge loan and your spouse has a huge income, it might benefit to file separately. This is not the case for you.
