Fortunately, I've been able to control bleeding with simple pressure and/or pressure dressing until arriving in the ED throughout my career. Can't recall a time when bleeding was "out of control".

worst bleeders are of course on coumadin and they can have endless oozing, but still nothing that a bulky pressure bandage and a trip to the ED couldn't fix (ie needs Vit K, FFP etc...)

It is exothermic, and can damage the surrounding tissue. Has use for wilderness medicine where time to surgery might be long, but in the short term it is relatively unproven against traditional methods of hemorrhage control. I would pack it if making a "survival kit" and see a role for it in tactical EMS but I don't see it on ambulances, if for no other reason that EMS uses "toys" if they have them and I can't see that the times this might work would outweigh the risks of inappropriate usage.

Interesting. I've never had a time where I couldn't control a bleed with direct pressure +/- pressure points, but I only have a couple of years of experience so far...Thanks for the response.
 
I've been doing EMS for going on almost 9 years, and I have only had one case of external hemorrhage that I have not been able to control with traditional methods. That case was a 22 m/o kid with a previously undiagnosed clotting disorder (I don't recall which one).
 
We used quickclot to control bleeding on a pig we opened up(class experiment)in class. It was crap, we used the whole container and it eventually stopped but after a ton of blood was lost.
 
is this like d-stat? if so...we used this for all of our balloon pump removals in ICU for a couple months. next thing i know, it's no longer allowed. cons outweighed the pros i guess.
pretty soon we won't be able to used our fem stops. problems with pts with peripheral vasc issues.
