I came across info for "Racz Lab". I realize that it will involve a lot of on the fringe Raczian procedures and pushing Epimed products. Has anyone attended one of these?

They claim that the Racz lab is more than just the Racz procedure. Although I am certain that they will show all of the interlaminar and transforaminal cervical and lumbar lysis procedures because that sells Epimed catheters. I'm not terribly interested in the lysis procedures as they are way too dangerous in my mind. The fee is VERY reasonable. Thought it MIGHT be worth checking out.

Has anyone ever seen good results from lysis. I did it initially when I was right out of fellowship but no one really got that much more benefit than they would from a plain old epidural. I eventually gave it up and have been going straight to SCS for my FBSS patients that have ongoing radicular pain not amenable to epidurals or further surgery
 
I haven't. I don't like it when they are on my schedule. I don't even think they are fun. I think there is probably something to be said about a catheter guided steroid injection - but that is probably the only good benefit.

Also, we don't do them correctly. I just do hyrdodisection with saline - no mechanical lysis. Also, one should use hypertonic saline.

I have never used hypertonic saline. The potential complications scare me. I do a gentle mechanical lysis on the very rare occasion that I do a Racz procedure.

Yes, I do a gentle mechanical lysis with NS and plenty of contrast. Then marcaine and Celestone.
 
Epidural adhesiolysis has been eliminated by Noridian medicare since late 2015 in western region.

no, have not done them since.
 
I dont think I've ever done them the "complete way"; hypertonic saline, epidural "flossing," inpatietn admission, wydase injection, etc. I dont understand how they get this covered with insurance.
