Freestanding community has been advocating it for well over a decade also. ASTRO only recently came around to it

Don't really see it ever happening, esp if hospitals find out they'll get paid the same case rate as a freestanding center. Plus the proton lobby etc.

• Fewer fractions per patient (due to hypofractionation and guidelines) reduces revenue.
• Reimbursement per fraction is decreasing with CPT code revisions and prior authorization hurdles.
• Consolidation limits negotiation power, keeping salaries stagnant despite rising workload.

Then again, why would ASTRO leadership with involvement in a residency program say any of that?

EBRT would be ROCR'd and then Brachy would be FFS was my understanding. Otherwise lots of cervical cancer patients are going to die unnecessarily.

And yet, that's not how it was going to be in RO model (if I recall right, hospitals were expected to eat brachy boost costs)

I heard it from 2 sources now. I thought they were a decent mid tier program at least a few years ago

ACGME says they were put on probationary accreditation on 1/9/2025 with the next site visit scheduled for 1/1/2026.

Apparently they weren't the only program who did this

I think not SOAPing allows them to possibly offer the spots outside of the match during the year

I'm guessing there were a lot more than 5 unfilled slots this year when you consider programs that chose not to soap but will offer spots out of the match

I can’t imagine that mid tier academic training programs have been particularly pleasant places to work or train. At least in the past there was a light at the end of the tunnel and there was a chance you could work with some bright imaginative faculty and residents…no more it seems.

Interesting, UVA actually didn't participate in the match. VCU was the 4th that didn't match, and appears not to have SOAPed, but in the past they have just used them as reserved spots in the following match.

What are we going to do about the obvious future shortage of radiation oncologists in the state of Virginia that these closures will undoubtedly cause???

this interview likely predates the official decision to close the program. UVA will be back. ACGME was likely going to withdraw accreditation, Voluntary withdrawal allows a quicker timeline to remediation and re-accreditation.
