Hi I am taking the MCAT again in august. I currently have a 27 on the mcat and a 3.8 GPA. I was wondering if I should submit my amcas prior to taking the mcat in august and mark that I am taking the mcat again or if I should wait.

This will be my second time apply to medical school. Any help would be great.

Submit now. Mark that you are retaking. You'll want to get your AMCAS verified earlier.

Also if I have the opportunity to shadow a doctor even though it is late should I still take it? It is something I could discuss if I get a interview.

Yes, you should. You can always write about it in an update letter or an interview.
