Really struggling with Why DO secondary

Basically title. I would like some tips or structure of what to follow. I have a draft where I like the two first two paragraphs but I feel like I kinda drift out to space. any help would be appreciated

Everyone i've talked to said they basically treated it as a "Why Medicine", but they also applied to MD so maybe thats why LOL
