How worried should I be at this point in the cycle? What can I do better for next cycle?

Your lack of physician shadowing and non clinical volunteering (besides tutoring) was a factor in not receiving more interviews. Accumulate 40+ hours of in person physician shadowing and 150+ hours of non clinical volunteering such as food bank or homeless shelter before you reapply. There will be significant waitlist movement starting late April. MCG would be your best chance.

What have you been doing where you're in the room with doctors? What fraction of your hours does this involve? What type of clinic are you working, and what type of doctors work there?

I work at a PM&R practice and I’m the MA for a specific doctor. We’re in clinic with patients all day. In addition to new patient and follow up appointments, we do various injections (US and XR guided) and I control the C arm. I’m in the room with every patient.

Genuinely asking: how would shadowing be any more meaningful? I have shadowed a pediatrician for a day but I think my job is more involved than shadowing

How did you describe your shadowing? Looking at the number of rejections without interviews, it seems many of the adcoms don't agree with your position. I would agree that your MA should also count, especially if your physician attests in your LOR.

But you did get 3 interviews with two pending answers. It's smart to plan your reapplication, but you could get an offer too. We'll see, but trust the process.
