Reapplying after failing level 1 exam

I was dismissed two years ago from my osteopathic school after failing the level 1 exam, along with an earlier course failure during my second year. For the past two years, I've worked as an EMT and a certified nurse aid, along with prior experience as a research tech before med school.
Currently, I am planning to reapply, and my overall undergraduate GPA was 3.25, with a MCAT score from 2014 of 31Q. Due to my lower GPA, I realize that my chances with allopathic schools are limited, so I am primarily planning to apply to osteopathic programs. How severely will my previous problems influence the chances for my application? Am I required to submit the information that I was previously enrolled, since I am not planning to transfer any credits?

There must be more to the story e.g. how many times did you fail Level 1, and how many times did you fail the course? And no, you cannot hide your previous med school enrollment. (And why are you asking premeds?)
 
Also you didn't get a score on the MCAT of 31Q in 2014. The writing was dropped by then. Your MCAT score is most likely expired and you will have to retake.

Your post raises too many questions. I will assume you made a mistake with the MCAT date since the writing section was dropped around 2013.

1) Did you have one board failure or three?
2) What does your course failure have to do with anything because you should be able to remediate or retake the year? Something strange here.
3) How did you approach the faculty on your possible dismissal?

You should be posting in the medical school forums, because you will get the best answer there. Bones DO was a difference case from yours since he failed one course ONCE and was dismissed. Most schools allow remediation or retake of the year, which he was not offered. If you failed your boards three times, this makes your chances radically decrease. If so, I really doubt you will get in and think you are better off pursuing something else.
