recognizing older siblings at med school...

on some secondaries they ask if a family member went to the school etc etc.
also maybe if it's a reason why you want to go to the school?

I'd say it depends on the school. You could just call their office and ask whether they're interested.

It's possible they'll know even if you don't tell them. My wife's at Minnesota, and when her younger brother interviewed last year he was surprised when the interviewer brought it up, because he hadn't said a word about it in his materials. This was doubly strange because she changed her name when we got married, and so they no longer shared a surname. He got in and just started last week.
 
What about dropping other names? 😀 My aunt went to a school I'd like to attend, and she's now a chief general surgeon in the area with a lot of name recognition.
