Recording shadowing time

Hi, I am new to this forum :zip:and I was wondering how one goes about recording the amount of time shadowed? I have only shadowed a maxillofacial surgeon in a private practice so far; however, I am hoping to begin shadowing physicians in-hospital soon and I would like my application to reflect the hours I put in.

You don't. Just estimate it to the best of your ability. The schools won't check anyway, they trust you.
 
You can record it on a piece of paper, or Word document, or whatever.

You will eventually need to know:
Time period for the shadowing of each physician (month is fine)
Total # of hours for that physician
Contact info (phone # or email) for someone who can vouch for you, in this case the office number will suffice

There is no further verification required
