What if I select no? Is this to check if i submitted my hours correctly or ask about me? I have nothing to hide when it comes to my experiences but I volunteer at a couple of places where I'm not that familiar with the staff. so I don't think they would have something to say about me.

I don't think it matters that much. Of course it depends a little on the activity I would say. For example, you are a club leader of your extracurricular club... who are you supposed to put as the "supervisor contact" for something like that? maybe if said activity was like working at a job then you might want to put the supervisor down if you ask them for a reference.

Thanks guys! another question, how far back do they want us to go back on work experience? should I include the places I worked at before starting my freshman year in college or only the jobs i had during college?

Just jobs you had during college. What you did in high school doesn't count/matter.
