Your chances for interviews are limited since you have no non clinical volunteering, only 20 hours of shadowing and no clinical exposure other than research. In the coming year you should accumulate 150+ hours of non clinical volunteering such as food bank, homeless shelter, etc. as well as another 30 hours of physician shadowing (including primary care). I’ve been considering doing more volunteering on weekends or at the end of the day, but I’m worried that if I start doing that now wouldn’t it look like I’m just checking off a box? For shadowing that’s difficult since I work 9-12 hours a day during the week.
 
Why aren't you applying to HBCU schools?

Could you explain your community service and your shadowing?
I forgot to add that I applied to Howard after submitting my first round of primaries.

I don’t have much for community service unless you count being a transfer student mentor. I also volunteered at a neurotrauma conference (same one that I presented at) where I helped neurotrauma survivors and other attendants navigate the conference, and helped conduct lectures (probably was around 20-25 Hours).

My shadowing was with my neurologist a few days during summer classes and research.

You should accumulate the hours in the coming year in case you need to reapply. At the schools you applied to, you will be competing with applicants with the same stats as you but with hundreds or thousands of hours of clinical volunteering/employment and non clinical volunteering. Since you are a Georgia resident why have you not applied to Medical College of Georgia and Mercer?

Will do, if I do end starting some clinical volunteering and non clinical volunteer this fall do you think it would be worth to send these schools and update letter stating this or would it come off bad.

Also I decided not to apply to those schools since I really value doing clinical rotations in a relatively large city that could expose me to wider range medical conditions and patient populations (Yale & Brown is an exception to this due to their research), and research resources and opportunities. I also don't have a vehicle to go to some of their clinical rotations that are hours away.

Yes, send the schools an update when you have those activities and hours.
