Throw clinical training out the window, what are the rankings for fast track and/or research track programs? Well don't throw the clinical work too far out the window.... but let's focus on physician scientist academia training.

It's got to be the following in some order:
Hopkins
MGH
Brigham
Penn
Yale
Cornell
Stanford
?Duke
?WashU

I'm not sure there's a hard and fast ranking for research programs given that much of it depends on the fellowship and what sort of research you're interested in doing at the place. For example, I'd argue UW, Pitt, or Colorado are better research programs if you plan on doing a pulm/cc fellowship and have an identified mentor at those places vs. going to a UCSF or Columbia or even the Boston programs. On the residency front, the last 3 are higher ranked IM programs, but the fellowship and research may not be the same (or it could be if you have someone there you'd like to work with).

The general advice our MD/PhD faculty keep giving us is to focus on the fellowship part, ensure there's adequate support for research (as in make sure the fellowship has a training grant), and that there's a good research group to join.

If you are interested in a particular sub-specialty...make sure to look at the T32 grants on eReporter (NIH.gov....know it and love it).

Also good to look at division R01 funding and PPG funding

If you are a MD/PhD and you know some older grads who did ABIM programs- CALL THEM.
I found this to be VERY useful.

I'd say Wash U is the best RESEARCH-track (aka fast track) program on that list as it has one of the best track records for mixing the training well. Hopkins, MGH, Brigham, Yale, and Cornell do not. I interviewed at all of those places.

The very best reseach track residency I saw was Vanderbilt. Hands down. It was the best, the most organized, and combines a very strong IM experience with top-flight research.
