I'm not really close to applying to med school, but I was just wondering, how do med students/residents live with all of the debt they accumulate? What kinds of things do you have to do without?

Medical students have loans that cover their needs until after residency when they begin payback.
They can also begin payback in residency if they wish.
There are also many ways to reduce debt depending on one's specialty and practice location.

You have to live without living above your means. Just because you graduated med school doesn't mean you are a doctor, both in terms of full practicing ability and paycheck/lifestyle.
 
PAYE payments are close to $250/mo. You're bringing in $50k/yr as a resident, which is like $35k/yr after taxes and benefits.
