I went through residency prior to the 80 hour work week and things were not good. So now that there are only 80 hours, how many patients do you think you see in a typical week?

And a case from last week: what do you think of a weird guy in the ER, PMH of chronic renal issues, HTN, with numbness in all 4 extremities for 2 days, ratchety weakness in his arms and trivial weakness with poor effort on exam. He has sensory loss in a patchy distribution, good reflexes in the arms, normal patellars, no ankles. His gait is wide.

The ER wants to send him home.

Any alcohol history in the patient?

No, but he likely had pre-DM.

In the ER with a few days of paresthesias and subjective weakness, what's the worst thing the guy could have? Other than fibromyalgia.

So how hard do you work during those 80 hours.

.

Multiple sclerosis?

Well, I LP'd him and he had a protein in the 90's and nothing else.
