Nah not at all. That experience definitely counts. If you want you can do some volunteering or other clinical thing to be able to say you've kept up doing clinical things, but just because your last shift was Jan 2021 doesn't mean it wasn't a meaningful clinical experience that informed you of medicine, which is the whole point.

You don't "age out" of clinical experience.

But, consider how you would answer the question of why you haven't been involved in clinical work during the past 3 years? It could certainly suggest to an admissions committee that you aren't really that invested in pursuing clinical practice.
