We are experiencing the 0.75% bupivacaine with dextrose shortage, and now pharmacy has said they are running low on 0.5% bupivacaine. Have heard of ropivacaine spinals, but never seen or done one.

What is the usual dose? Did a search, and looks like around 20 mg. Is anyone doing these, and if so, what dose are you using, and what are you getting in terms of duration? Thanks!
 
Surprised you guys are still experiencing shortages. We’ve recently started to get stock back including heavy bupi. Seems as though all the Puerto Rico factories are back online.
 
A month ago did 2.5cc of 0.5% rop for bilateral lower extremity vascular nonsense on a 65kg guy. No fent. Lasted 2.5-3 hrs. Did take longer to set up and I don't think the motor block was as good but it got the job done.
 
I have performed around 20 during the bupivacaine shortage. The longevity of the block is 2/3 that of bupivacaine but can be extended with epinephrine or 5mcg precedex added to the block. It does take a bit longer to set up compared to spinal bupivacaine, but about the same amount of time as isobaric bupivacaine. Usually used 15-25mg ropivacaine based on height. No block failures.
 
We used isobaric 1% Ropivacaine for a period of time due to no access to Bupi. Doses between 15-20 mg. As above, a little bit longer to set up and motor block not as dense.
