Pick 2 out of the three electives in terms of usefulness - ID, Cardiology (with cardio team/CCU), Critical care (MICU). I like all three areas equally. I am interested in doing a residency. Internal medicine is required rotation that I have to take. Can only choose two acute care electives. The cardiology preceptor is great from what I heard. I have no idea about ID and critical care preceptors/sites.
 
To pass NAPLEX, the best choices would be cardio and ID.
 
I would say critical care and ID. There is more need now for PGY2s in ID than cardiology, so it's an area worth exploring, and you'll use your critical care knowledge a lot in multiple areas of residency. I still wish I had more critical care experience (and I've been a practicing pharmacist for 2.5 years if you include residency).
