Are there residency programs that are more focused on rural medicine to the point that you may not need to do a rural medicine fellowship? I imagine that would be some of the unopposed ones I have read mention on this forum, but I'm not sure.

there are plenty! Klamath falls OR, Roswell NM 1+2, Montana, Wyoming, unopposed ones in CO (all CO programs require a rural month).
 
I did see some of those areas listed on AAFP. They look right up my alley. But I also saw that some school programs have a rural fellowship also so I wasn't sure if it was necessary. I figured if you go to the right residency that prepares you properly then you might not need that fellowship.
